Personalised support at home

At Compassionate Healthcare Jersey, we provide home care that is practical, respectful, and centred around the individual. Our services are designed to help adults live safely and as independently as possible in their own homes, with support that reflects their routines, preferences, goals, and wellbeing.

We know that no two people are the same. That is why our care is built around what matters most to each person, with support planned carefully and reviewed regularly so it remains safe, appropriate, and responsive as needs change

Personal Care

Our personal care service supports people with the everyday aspects of care that are closely linked to dignity, comfort, and wellbeing. This may include help with personal hygiene and other agreed routines, always delivered in a respectful and sensitive way that protects privacy and promotes confidence.

We understand that personal care is deeply personal. Our approach is gentle, person-centred, and shaped around the individual’s wishes, so people feel listened to, comfortable, and treated with the dignity they deserve.

Personal Support

Our personal support service helps people remain safe and well at home when they need extra help with day-to-day life. Support is tailored to the individual and may be provided on a regular planned basis to help maintain stability, comfort, and independence.

We work in partnership with the person and, where appropriate, their family or representative, so support feels joined up and meaningful. The aim is always to provide the right level of help without taking over more than is necessary.

Daily Living Support

We provide practical help with daily living tasks that support safety, wellbeing, and everyday comfort at home. The SOP specifically refers to support with daily living tasks and life skills such as preparing simple meals, managing household tasks, and attending appointments and activities.

This kind of support can make a real difference to someone’s confidence and quality of life. We want people to feel that life at home is manageable, settled, and supported in a way that respects their independence and routines.

Independence and Wellbeing Support

A key part of our service is encouraging people to do as much as they can for themselves, with support provided only where it is needed. Our care is designed to promote independence, maintain daily living skills, and help people remain as active and involved in their own lives as possible.

We also recognise that wellbeing is about more than physical support. Emotional wellbeing, reassurance, and feeling respected in your own home all matter. That is why our care combines practical help with a calm, compassionate approach.

Appointments, Activities, and Community Access

Where appropriate, we support people to attend appointments, maintain routines, and stay connected to life in the community. The SOP also says support may include encouraging participation in community activities, supporting hobbies and interests, and helping people access local services and groups.

Our role is not to run activities services directly, but to support access, inclusion, and confidence. For many people, this kind of support can help reduce isolation and make everyday life feel fuller and more connected.

Companionship and Social Connection

Good home care is not only about tasks. It is also about human connection. We recognise how important family, friendships, conversation, and social contact are to a person’s wellbeing, confidence, and sense of belonging.

Our support can include helping people maintain contact with family and friends, attend social and community activities, and continue the relationships that matter to them. With the person’s consent, we also work positively with families and carers so care supports respectful and supportive relationships.

Short-Term and Respite Care

We can provide short-term or respite care for a limited period when extra support is needed. This may be helpful during times of change, recovery, family pressure, or when regular carers need temporary relief and reassurance.

These packages are reviewed regularly to make sure the support remains right for the individual. Depending on needs, support may continue, reduce, or come to an end once the short-term period has passed.

Planned and Urgent Support

Most care is arranged in advance through a planned rota. Wherever possible, the service aims to provide continuity of staff and consistent visiting times, because familiar faces and reliable routines can make a big difference to comfort and trust.

The SOP also allows for urgent support where staffing capacity permits, such as after a sudden hospital discharge or an unexpected deterioration in condition. Outside office hours, an on-call manager is available to help manage urgent situations and liaise with professionals where required.

Every care journey begins with a simple conversation. We take time to listen, understand what support may be needed, and answer any early questions in a calm and reassuring way. This first step helps us decide whether our service is the right fit and whether we can support the person safely and appropriately. We want the process to feel clear, respectful, and unhurried from the very beginning, so people and families can move forward with confidence.
Understanding You
A Person-Centred Assessment
Following assessment, we agree a personalised care and support plan with the individual and, where appropriate, their family or representative. This sets out the type of support to be provided, how often visits will take place, how risks will be managed, and the outcomes the person wants to achieve. Care plans are agreed and signed before support starts. We believe good care should always be thoughtful, clearly planned, and built around the person’s own routines, preferences, and priorities.
The Right Match
Planning Safe and Consistent Care
Once support is agreed, we plan visits carefully so care feels reliable, respectful, and consistent. Wherever possible, we aim to provide continuity of staff and regular visiting times, because familiar faces can make a real difference to comfort and trust. Staff are matched to care packages based on the tasks involved, the person’s preferences, the location, and the level of support required. Our aim is to provide care that feels settled, safe, and well organised from the start.
Our Team
Staff Training and Supervision
Great care depends on great people. Our staff complete induction before working unsupervised and are supported through regular supervision, spot checks, and performance review. Care is delivered by trained Support Workers under the oversight of the Registered Manager and Care Coordinator, and staff only carry out tasks they are trained and assessed as competent to do. This helps us maintain safe practice, clear accountability, and a consistent standard of care that people and families can rely on.
Growing Stronger
Continuous Learning and Development
We believe learning should never stand still. As our service grows, we continue to build staff knowledge, confidence, and competence through ongoing training linked to the needs of the people we support. Senior staff receive additional leadership support, and training compliance is monitored as part of our quality arrangements. We want our team to keep learning, reflecting, and developing, so the care we provide remains safe, effective, responsive, and centred on each individual’s wellbeing.
Your Voice Matters
Listening to Client Feedback
We actively encourage feedback from the people we support and from their families. Feedback can be shared through conversations, review meetings, surveys, and regular check-ins, and it is followed up so people feel listened to and respected. We use what we hear to review care, improve quality, and respond when something needs to change. In our view, good care is never one-sided. People should be active partners in shaping the support they receive.
Clear Boundaries
Care Within Our Registered Scope
Compassionate Healthcare Jersey provides non-nursing home care only. We focus on personal care and personal support for adults in their own homes, and we do not provide nursing care, invasive procedures, residential accommodation, or continuous 24-hour medical supervision. We also begin as a general home care service rather than a specialist provider. Where needs become more complex, support will only continue if it can be provided safely within our registered scope, training, and competence.
Ongoing Support
Regular Reviews and Trusted Care
Once care is in place, we keep it under regular review so it stays right for the person receiving it. Care plans are reviewed at least every six weeks, or sooner if needs change, and ongoing assessment is supported by daily records, supervision, spot checks, and feedback from the person and their family. Our aim is to provide support that remains safe, responsive, and reassuring over time, while continuing to promote dignity, independence, and wellbeing.

Training Modules

Induction and Mandatory Training

Before working independently, all new staff complete a structured induction programme. This introduces the values of the service, person-centred care, communication, confidentiality, safeguarding, professional boundaries, and the standards expected within Jersey’s regulated care environment. Mandatory training then covers key areas such as safeguarding adults and children, health and safety, infection prevention and control, fire safety, moving and handling, food hygiene and nutrition awareness, medication administration, equality and diversity, data protection, and first aid with basic life support.

Specialist and Enhanced Training

We know that people’s needs can vary, so training does not stop at the basics. Our team also completes additional learning in areas such as dementia awareness, learning disability support, positive behaviour support, autism awareness, mental health awareness, trauma-informed care, reablement, promoting independence, de-escalation, and end-of-life care where appropriate. This helps us build confidence, broaden understanding, and make sure support is delivered with sensitivity, competence, and respect.

Ongoing Supervision and Development

Learning is an ongoing part of working in care. Staff receive regular supervision, reflective practice, spot checks, and annual appraisals to help identify strengths, support development, and maintain high standards. Competency assessments are repeated periodically, and training compliance is tracked and audited to make sure staff remain up to date. Senior staff also receive additional leadership support, helping us maintain a service that is safe, well managed, and committed to continuous improvement.
